Step 1 - present the necessary requirements to the enrollment adviser
NEW STUDENT - brown envelope and long folder with entrance requirements and result of physical exam
OLD STUDENT - latest clearance, physical exam result
Step 2 - REGULAR students - fill out registration form at the designated enrollment area. To be countersigned by the enrollment adviser.
IRREGULAR students - proceed to the bulletin board. Using the pre-registration form, get the schedule of the subjects you are advised to take. Submit properly filled out forms to the enrollment adviser for his/her signature.
Step 3 - present registration card to the Accounting Office for assessment/ payment of fees
Step 4 - for new students, submit 1x1 colored picture to the Frontliner for processing your ID
Step 5 - proceed to the Realty Office at the 3 rd floor (hospital building) for payment and measurement of needed uniform
Step 6 - Present official receipt of tuition fees, uniform, ID/ receipt of ID at the Registrar's Office for issuance of class cards
Step 7 - Update your library card (for old students). For new students, submit one 1x1 colored picture at the library for issuance of a library card.
NOTE:
No student will be allowed to enroll in a subject until he/she has satisfactorily passed the prerequisite subjects.
Block system/section is implemented.
Refer to Bulletin Board of your respective department for schedule of classes.
Adding/Dropping/Change of subjects will be entertained within two (2) weeks after the beginning of classes. (late dropping – NON-REFUNDABLE)
Enrollment time – 8:00 AM – 12 NN and 1:00 PM – 4:00 PM – Monday to Fridays
A student is officially enrolled when:
a. He has submitted the appropriate admission or transfer credentials.
b. He has made an initial/ full payment on his school fees.
c. He has been advised to attend classes.
