FRESHMEN

Fill out application form at the Front Liner.
Proceed to the Guidance Center for schedule of entrance examination
Pay entrance fee to the Cashier
Present receipt of payment to the Guidance Center for entrance examination.
Get request slip for physical examination at the Front Liner
Proceed to Delos Santos Medical Hospital Cashier for payment of physical examination
Get result of the entrance examination and personal interview at the Guidance Office
Submit admission requirements to the Registrar’s Office together with the result of Entrance examination, personal interview and physical examination result.
Follow enrollment procedure.

Submit photocopy of Scholastic Records of Official Transcript of Records to the Registrar’s Office for evaluation.
